Children and youth with special health care needs can get free resources, advocacy, and referrals.

Eligibility

To be eligible for services, the child or youth must:

  • Have special health care needs
  • Be 21 or younger
  • Live in NYC

Special health care needs include serious or chronic health conditions or disabilities.

These conditions can be:

  • Physical
  • Developmental
  • Intellectual
  • Emotional
  • Behavioral

Examples include autism and childhood disorders such as ADHD.

Services

An intake specialist will assess your child’s needs and help you find health care and community resources. They can also help you get evaluations for school and apply for health insurance.
