You can get a copy of an immunization or vaccine record for yourself or your child using My Vaccine Record. You will need a valid form of identification.

Printouts from My Vaccine Record are official reports that you can use for school, college, or camp enrollment. You can also show it as proof of COVID-19 vaccination.

Records You Can Access

My Vaccine Record only has records for immunizations that were reported to the Department of Health and Mental Hygiene (DOHMH) by NYC health care providers.

NYC health care providers must report:

  • All immunizations given to children 18 and under in NYC
  • All COVID-19 vaccine doses given in NYC

Most records are for people born in NYC after 1995. Only some vaccinations given to adults ages 19 and older are reported, so records may not exist or be incomplete.

Who Can Access Records

You can access your own immunization record if you are 18 and older.

To access your child’s records, you must be listed on the child’s birth certificate or immunization record as the child’s:

  • Parent or guardian
  • Next of kin
  • Primary contact

You can access records in My Vaccine Record using your:

  • Phone number
  • Email address
  • IDNYC number

NYC Health Department Vaccine Record

If you received the COVID-19 vaccine in NYC, use My Vaccine Record to get a copy of your vaccination record. This is not a replacement for the CDC COVID-19 Vaccination Record Card. It is an official record noting you have been vaccinated.

COVID-19 Vaccination Record Card

After you get the COVID-19 vaccine, you get a Centers for Disease Control (CDC) COVID-19 Vaccination Card.

The NYC Health Department doesn’t replace these cards. Use My Vaccine Record to get a copy of your vaccination record instead.

Excelsior Pass Plus

New York State’s Excelsior Pass Plus provides secure, digital proof of your COVID-19 vaccination status. 

As of July 28, 2023, the Excelsior Pass Plus and the NYS Wallet Apps are no longer available.

You can still open the app and use it to prove that you received the COVID-19 vaccine. However, if the app fails, technical support will no longer be available. In addition, since the app will be removed from app stores, new users are unable to log-in and register for it.

NYC COVID Safe App

Some New York City employers, businesses, and venues require proof of immunization for their employees and customers. You can use the NYC COVID Safe App to store and present the necessary documentation.

The documentation you upload into this app is not shared with anyone and stays on your phone only.

The Citywide Immunization Registry (CIR) keeps a record of immunizations that were given to children 18 and under in New York City.

CIR also has records of vaccines given outside of NYC if a provider reported those vaccinations to the NYC Health Department. Note that only some vaccinations given to adults ages 19 and older are reported, so records may not exist or be incomplete.

Complete the online Record Assistance Request Form and upload a copy of your valid photo ID. Staff will respond as quickly as possible, usually within 2 to 3 days.

Mail or fax a completed Immunization Record Request Application with a copy of your valid photo ID. Requests take about 1 to 2 weeks to process.

If you need a copy of a vaccine record right away and can’t use My Vaccine Record or mail or fax a record request application, send an email to NYCvaxrecord@health.nyc.gov. You must explain the reason for the urgency or emergency. 

You should also include in your email any supporting documentation showing the reason why you need your vaccine record right away.
